Arbor Dental Associates ("we," "us") runs this website for patients and families in Novi and nearby communities. This policy explains what the website collects and how we handle it. How we protect your health information as a patient is governed by our HIPAA Notice of Privacy Practices, available at the office and on request.
The short version. The website collects what you enter in the appointment-request form plus ordinary server logs. No advertising trackers, no analytics. Please keep detailed health information for the phone or your visit, not the web form. We don't sell personal information.
1. What the website collects
- Appointment request — your name, phone, email, the kind of visit you're looking for, preferred days and times, and any notes you add. Requests are stored on our hosting platform and emailed to our front desk so we can call you back. We also record the date, the page the request came from, and your device's IP address and browser type for security.
- Form protection — the form uses Cloudflare Turnstile to stop spam and limits how many requests can come from one address per hour.
- Server logs — the site is hosted on Cloudflare, which records requests (IP address, browser type, pages requested, timestamps) to serve the site and block abuse. We look at these only in aggregate.
- Fonts and scripts — pages load type from Google Fonts and one animation library from the jsDelivr content network; those services receive your IP address when the files load. Links to Google Maps and Google reviews take you to Google under its own policies. We don't use Google Analytics, advertising pixels or cross-site tracking, and the site sets no cookies of its own.
The appointment form is not a secure channel for medical details. Tell us the basics of what you need and we'll take the rest by phone or at your visit. Anything you do include is treated as confidential and used only to schedule your care.
2. How we use it
To respond to your request, schedule and confirm appointments, answer questions, and operate and secure the website. We don't send marketing messages through the website form.
3. Who we share it with
Only the service providers that run the site: Cloudflare (hosting, storage, form delivery and bot protection) and, as described above, Google and jsDelivr for fonts and scripts. Our website developer maintains the site under a confidentiality agreement. We may disclose information when the law requires it or to protect someone's safety. We don't sell personal information.
4. Retention and security
Appointment requests are kept as long as needed to schedule and follow up, then removed from the website system; your patient record is kept separately under our records policies. The site is served over HTTPS and access to stored requests is limited to staff who need it. No system is perfectly secure — please call us if you think information you sent us has been compromised.
5. Your choices
Call (248) 553-9393 or visit the office to ask what website information we hold about you, correct it, or have it deleted. Rights over your health records are described in our Notice of Privacy Practices. Residents of states with consumer-privacy laws may have additional rights, which we honor regardless of where you live.
6. Children
We treat patients of all ages, but the website is for adults. Appointment requests for a minor should be submitted by a parent or guardian.
